Page one and two should be completed by the student and parent prior to seeing the physician. Only the Last page (MSHSAA Medical Eligibility Form) will be accepted by the school. We will no longer accept any other form from a doctor.

MSHSAA requires our Athletic Department to follow a return to play procedure when a student is diagnosed with a concussion. Our Athletic trainers have taken the process a few steps further to even further protect the health of our student-athletes.

Permission Slip for Students to Drive
