Page one and two should be completed by the student and parent prior to seeing the physician. Only the Last page (MSHSAA Medical Eligibility Form) will be accepted by the school. We will no longer accept any other form from a doctor.

Use this form and turn it into the coach if a student needs to transport themselves to and from an away athletic contest. The Coach will make final approval on the driving situation.
